Our knowledge of monitoring and providing recommendations to treat our patients’ medical conditions to allow them to participate in rehab safely and with stability decreases re-admissions to acute care. Communicating with appropriate medical attendants and specialists involved in a patient’s care helps to ensure patients optimize their potential in therapy and achieve their rehab goals.

We believe patients should be seen weekly at skilled nursing facilities. We provide a Board Certified Physical Medicine and Rehabilitation Physician and Physician Assistants / Nurse Practitioners to lead your interdisciplinary team in creating appropriate and timely rehabilitation plans for patients. We provide an initial evaluation and are involved in the care of patients throughout their entire stay.

In the coordination of care between all members of the interdisciplinary team, we assist in ensuring patients achieve their rehabilitation goals and have an optimal discharge plan. We help patients and their families understand the medical and rehab care they receive throughout their stay, so they are well educated about their progress and prognosis in therapy. By assisting in the communication between the facility staff and patients with their families, we ensure the patients are highly satisfied with their medical care, rehabilitation and overall stay at the Nursing Facility.
